This form will be printed and hung on the front of your dog’s kennel in Adrian (Ari) Wichert’s home during your dog’s board and train stay. Please ensure the accuracy of the information on this form, as it will be referred to on a daily basis as well as in the case of an emergency in order to confirm important medical and training information.

For Puppy Prep students whose puppy is going right into the board and train program from the breeder, please indicate what food/supplements you want your puppy to be on, and they will be purchased in advance by Ari and added to your invoice. Do not worry about portions/feeding schedule, or cues list.

What is your dog's daily food intake? Please note that due to factors such as body condition score and how much exercise your dog is used to, they may eat more or less during their stay with Ari than what you usually feed them.
